Initial Comments:
Based on the findings of an offsite followup survey completed on October 25, 2019, From The Heart Too, was found to have corrected the deficiencies and be in compliance with the requirements of PA Code, Title 28, Health and Safety, Part IV, Health Facilities, Subpart A, Chapter 51. The deficiencies were cited as a result of a State relicensure survey completed on January 22, 2019.

The deficiencies were cited as the result of a State relicensure survey completed on January 22, 2019.
